EXPANDED
SCHEDULE Since
about half the firms opted for some sort of expanded program
last season, the League is again offering the following additional
options. Please check those that interest your firm and remit
the charge indicated along with the entrance fee.
LEVEL OF
PLAY In an attempt to create
a competitive balance at all levels of play, teams are placed
in divisions that are hopefully fair to the specific team involved
and the League in general. In order to assist us please answer
the following questions?
What division
where you in last winter? How did you do?
Please indicate
if you have added or subtracted any key players (with name) and
compare your team to last season's.
Accordingly,
in what division do you feel you would be most competitive this
season? (Note: Your choice is not binding on the
League) Check one:
LOCATION If you have a preference as to the area
of Manhattan you prefer to play most of your games, please place
a check below and to the extent possible we will accomodate
you. Please
check two.
STARTING
TIMES Given that
the League cannot accomodate all teams preferences all
the time and that virtually everyone prefers middle evening games,
please indicate if you would prefer the majority of your games
to start either early or late (with the balance starting in the
middle starting slots). Please check one.
DAYS OF
THE WEEK Games are
scheduled Monday through Friday. Note: All teams
will be scheduled for one, possibly two, Friday game(s) in order
to compensate for not playing on Thanksgiving, Christmas week
and New Year's Day. Are there any days of the week (Monday -
Thursday) when your team is NOT able to play.
Reason:
CONFLICTING
DATES Are there
any specific dates during the season when your team is NOT able to play due to conflict - such
as a company event, etc. (type date and list reason(s) below)

UNIFORMS Please note that League
rules require each team member to wear the same style (T-shirt
or reversible, but not both) and exact color (of both jersey
and print), no mixing. All uniform jerseys will have numbers
(on front and back), your firm name (if desired) and the League
logo. When ordering, please request a sufficient quantity for
all participants, as well as any extra jerseys that you might
need for partners, spouses, kids, friends, etc., as it is difficult
and expensive to fill piece-meal orders. All jerseys are a full
extra-large cut. Minimum
order is six.
If this is a re-order, please
specify the colors of last year's uniform.
TEE SHIRTS - $15 Choose your top three preferences. Place
a 1, 2 or 3 next to the color. (Note: League will
make final choice in order to eliminate duplications in the same
